Swans: Incident Report Sheet

A pair of dead swans found in a lake at a fishing, hunting, and skeet shooting club;

Directions: Please PRINT the following questions. Your  President and CEO (Your Teachers) will assign responsibility for answering the questions.  After composing your answers, please EMAIL  them to your manager and the President and CEO (Your Teachers).

Please note that while these incidence sections are completed independently, feel free to consult with your fellow team members as well as your President and CEO.

Answer the following questions:

1. What is the connection of observations of the symptoms of the swans to exposure to lead?

2. What are the human/animal activities and environmental conditions connecting exposure to lead to the observed symptoms?

3. Give a brief analysis of the geographic information supporting the exposure (refer to specific urls if necessary).

4. Give a brief analysis of the available data (soil, blood, dust, water, sediment etc.) supporting the exposure.

5. Briefly summarize your conclusions about the probable causes.
